## Statement

SGT Cummings responded to a vehicle accident. He submitted a Police Traffic Collision report omitting passengers, eyewitness and vehicle placement he observed. He also submitted the Lexus' wrong registered owner information. All in violation of the WA State Police Traffic Collision Report Instruction manual. SGT Cummings' Officer Worn Body Camera Video shows that he was aware of the passengers, eyewitness and Lexus' true registered owner. Neither SGT Cummings or the DPD made any effort to obtain passenger info or eyewitness statement and info. They instead wanted for one of the driver's spouse to provide it. On the accepted report they still listed the wrong registered owner and lied about where SGT Cummings observed the vehicles.
