Use these forms to submit an individual for review, file a documented complaint against an officer, prosecutor, judge, or government agent, or request the correction, modification, or removal of published information. Each submission is independently reviewed for identity, source reliability, procedural status, legal restrictions, and relevance to disclosure, credibility, integrity, bias, or the administration of justice.
Our training resources support everyone involved in the criminal disclosure process, including law enforcement, prosecutors, judges, defense counsel, public defenders and legal aid practitioners, and self-represented litigants.
